Licensed Practical Nurse

The LPN works in a self-directed manor with a team of individuals to manage a panel of patients in a primary care setting. He/she contributes to the nursing process through data collection and incorporates evidence-based practices to individuals and groups focusing on cost-effective ways to maximize wellness, prevent illness, and manage acute and chronic diseases. Nursing encounters may occur face-to-face or via telephone or other communication devices. The LPN demonstrates ability to educate patients, identify and link patients with appropriate resources, collaborate with the interdisciplinary team, perform clinical procedures, communicate well, and utilize the computer for electronic documentation, initiating orders per policy, data collection, and reports. The LPN may be assigned to cross cover in other clinical areas within their competency level.The following is a brief description of the total duties related to the position. Other duties may be assigned. The incumbent must be capable of demonstrating the following:
Technically proficient in initiating, performing, and completing assigned duties in providing care to variable patient populations. Ability to observe, identify and respond to the patient's needs for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors. Ability to perform, report, and record assigned nursing care done in the Primary Care settings, including but not limited to:
Obtaining and recording vital signs. Medication administration, including injections. Check expiration dates of meds and supplies prior to use Pain assessment, documentation and communication with medical providers Correctly collects, labels, prepares, and delivers specimens to the lab Document's patient's home blood pressure and/or blood sugar readings Simple Dressing Changes. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the National League for Nursing Accrediting Commission, 61 Broadway, 33rd Floor, New York, NY 10006. (1) Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment. (2) VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions. (3) Individuals granted a license by a jurisdiction that does not require graduation from an approved school. See paragraph 4, section C. Licensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN /LVN who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part. English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grade Determinations:
In addition to the basic requirements stated in this section, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. GS - 3:
LPNs at the GS - 3 grade level are expected to perform all duties within the scope of their license (e.g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved. There are no additional requirements to qualify for a GS - 3 LPN beyond the basic requirements listed above. GS - 4:
The requirement of a GS - 4 grade level is six months of qualifying experience as an LPN or LVN;-ORGraduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; -OR Graduation from an approved school of at least 24 months' duration. LPNs at the GS - 4 grade level performs all duties expected of employees at the GS - 3 grade level; however, they are expected to exercise greater judgment, require less supervision, and to operate in accordance with all established policies, procedure, and techniques. GS - 5:
The requirement for a GS - 5 grade level is at least 1 year of qualifying experience as an LPN or LVN at the GS - 4 level or equivalent. In order to qualify at the GS - 5 level, your resume must demonstrate your knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems; the ability to serve as a responsible member of the nursing team and interact with patients, family members, professional and supportive personnel involved in the delivery of patient care, incorporating acceptable,established customer service standards into practice; knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed medications; and the ability to recognize and appropriately respond to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies. GS - 6:
The requirements for a GS - 6 grade level is one additional year of qualifying experience as an LPN or LVN at the GS 5 level or equivalent. In order to qualify at the GS - 6 level, the applicant's resume must demonstrate the knowledge and ability to fully meet each of the performance requirements of the GS - 5 LPN/LVN in addition to the ability to:
1) appropriately carry out assigned patient care based on the patient's condition and use judgment in selecting the appropriate order and sequence of procedures and treatments; 2) accurately recognize, report and record relevant patient information; to recognized and consider emotional, cultural, spiritual, socio-economic, and age-related factors in identifying and responding to the patient's needs for care; 3) quickly recognize urgent or emergent patient care situations and initiate appropriate emergency interventions as directed by the RN and/or MD/DO; and 4) understand human behavior, patient motivations and reactions to situations, and appropriately use this knowledge in working effectively with patients, family members, and other staff, establishing constructive relationships to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills. A GS - 6 level LPN/LVN is expected to actively seek out educational opportunities to enhance nursing knowledge and skills, and be able to effectively share new knowledge gained with other staff to improve and advance nursing practice. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
